Page 1 of 2 Attachment C.5 Data Formatting All data should be submitted in a .csv (comma-delimited) format. The sheets in the workbook provide a template for how the tables should be designed. Data for all of the tables should be submitted on a periodic basis as specified in the instructions in individual tables. Formatting Instructions The required format for the entries in each column/field is indicated in the instructions for each sheet. Column entries in bold red indicate that the Company should enter only one of the possible entries listed for that column. No other entries should be entered. A date Text - YYYY_MM 2012_06 to 2015_08 The month and year for which the data is being provided "Internet Tier Transition Data" Table For each month beginning June 2012 and extending through August 2015, please provide the subscriber data by zip code requested in the table b l Column Variable Name Format Possible Entries/ Example Entries Description The table should include data on all of the Company's continuing Internet Access Service subscribers. A continuing subscriber is a person that subscribed to one or more Company Cable Services at the end of the previous month that still subscribes to at least one Company Cable Service at the end of the current month. The table collects data on the number of continuing subscribers that had a given Internet Access Service tier at the end of the previous month (begin_tier) that then had the indicated Internet Access Service tier at the end of the current month (end_tier). The table should have 49 entries per zip code and month and should include counts of the Internet Access Service subscribers that did not change their service tier from the previous to current month. Do not include Internet Access Service tier changes initiated by the Company at no additional charge to subscribers. E subs Integer 10 Number of subscribers that correspond to each Internet Access Service tier combination defined by Columns C and D in the zip code and month B zip Text 20427 Five-digit zip code (with leading zeros) for the service plan. (Use code 00000 for the Company as a whole) D end_tier Integer 0 to 6 The code for the Internet Access Service that the number of subscribers in Column E had at the end of the current month (0=Internet Access Service downstream speeds of strictly less than 3 Mbps; 1=Internet Access Service downstream speeds of greater than or equal to 3 Mbps but strictly less than 10 Mbps; 2=Internet Access Service downstream speeds of greater than or equal to 10 Mbps but strictly less than 25 Mbps; 3=Internet Access downstream speeds of greater than or equal to 25 Mbps but strictly less than 50 Mbps; 4=Internet Access Service downstream speeds of greater than or equal to 50 Mbps but strictly less than 75 Mbps; 5=Internet Access Service downstream speeds of greater than or equal to 75 Mbps but strictly less than 100 Mbps; and 6=Internet Access Service downstream speeds of greater than or equal to 100 Mbps) C begin_tier Integer 0 to 6 The code for the Internet Access Service that the number of subscribers in Column E had at the end of the previous month (0=Internet Access Service downstream speeds of strictly less than 3 Mbps; 1=Internet Access Service downstream speeds of greater than or equal to 3 Mbps but strictly less than 10 Mbps; 2=Internet Access Service downstream speeds of greater than or equal to 10 Mbps but strictly less than 25 Mbps; 3=Internet Access downstream speeds of greater than or equal to 25 Mbps but strictly less than 50 Mbps; 4=Internet Access Service downstream speeds of greater than or equal to 50 Mbps but strictly less than 75 Mbps; 5=Internet Access Service downstream speeds of greater than or equal to 75 Mbps but strictly less than 100 Mbp; and 6=Internet Access Service downstream speeds of greater than or equal to 100 Mbps) Page 2 of 2 Attachment C.5: Template for "Internet Tier Data" Table by zip code requested in the table below. Notes: In the table below, the first row indicates that in June 2012 there were 20 subscribers in zip code 20427 that had an Internet Access Service strictly less than 3Mbps downstream in May 2012 and still had an Internet Access Service strictly less than 3Mbps downstream at the end of June 2012. The second row indicates that no subscribers had an Internet Access Service strictly less than 3Mbps downstream at the end of May 2012 and then had an Internet Access Service of at least 3Mbps downstream and strictly less than 10Mbps downstream at the end of June 2012.
